The only acceptable QE request form is located in the link below, Appendix B QE Request Form. You must save the PDF and when opened as a PDF, enable all features to make the form fillable.

The Task Accomplishment record signature pages in the front portion of the PQS (Crew, Coxn, or PWO) for each candidate must accompany the QE Request form on the help desk ticket submission.
